Dental Insurance
Your insurance policy is a contract between you and your insurance company.
You are responsible for providing the most current insurance information. Patients who carry dental insurance are responsible for knowing the policies of their dental insurance plan and understand that all dental services rendered are charged directly to the patient/guarantor and that he/she, not your insurance company, is personally responsible for payment of all dental services. As a courtesy to you, this office will help prepare the patients insurance forms and/or assist in making collections from insurance companies and will credit any such collections to the patient's account. However, this dental office cannot render services on the assumption that our charges will be paid in full by your insurance company.
If our office is a participating provider with your insurance company, then the patient/guarantor is responsible for all deductibles and/or co-payments at the time services are rendered. Any amount reduced or not covered by your insurance company is the financial responsibility of the patient/guarantor. The responsible party will be sent a statement and payment of the remaining balance is due upon receipt. If our office is a non-participating provider, then payment of the full fee for the treatment/s provided is/are due at time serves are rendered unless prior financial arrangements have been made. As a courtesy to you, our office will submit the claim on your behalf. If the payment from the insurance company sends our office payment, then that money will be credited to your account and then refunded to you or applied to a future office visit at your request.

Patient Forms
If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
Payment for services is due at the time services rendered unless financial arrangements have been made in advance. Emergency and new patients may be required to make payment before the doctor performs an exam or treatment. MasterCard, Visa, American Express, Cash, and Check.
